Yes, if you decide with the birth mother she can include your last name on the original birth certificate. The adoptive parents will then later on receive another birth certificate with their names on it, but yours can be put on the original. The best thing is to speak to the birth mother and ask if she’d be willing to do that. The decision is hers.
